For the Federal Tax Return:
- We need the following information on each person to be included on your tax return. (This includes you, your spouses and each of your dependents. A dependent is a relation (child, step-child, adopted child, etc) whom you have paid at least half of their living expenses for the year. They must be 18 or younger or under 24 if they are a student. And to qualify as a student they must attend school full-time for at least 5 months out of the year.)
- Full name
- Social Security number or equivalent
- Date of Birth
- Your address and phone number.
- Drivers License number, issue date, expiration date and state for one individual.
- We need all the tax forms you have received.

- If you gave any financial gifts:
- If you or a dependent child is enrolled at an institution of higher education:
- If you own rental property that is not part of your home:
- If your rental is part of your home:
- If you have your own business:
- Some other possible information you might need to give us:
- Did you own or have control of a foreign bank account, trust or Financial Asset?
- Did you buy or acquire, sell or exchange any virtual currency?
- Are you considered legally blind?
- If you are married but do not want to file jointly, tell us.
- Please tell us if one of your children/dependents is disabled.
- If you are getting a refund, we need to know if you want the refund direct deposited or sent to you in a check. If you want direct deposit, we will need your bank routing number and account number.
For the Idaho State Tax Return:
- We need to know if all the individuals included in your tax return have lived all 12 months in Idaho. If not, how many months have they lived in Idaho.
- We need to know if you gave a financial gift or technology gift (computers, etc) to any schools in Idaho (including private schools).
